1. W-9 Form
   Complete the W-9 form using the link below:

• Ejection Form
  Anytime a coach or player is ejected, this form must be completed and submitted to Empire Sporting Events on the same day.

  For any incidents requiring documentation, please complete and submit this form on the day of the incident.

Rival has a zero-tolerance policy regarding abuse towards umpires. Please note the following:

• Coaches and managers may be ejected for their conduct or the behavior of their parents and fans.
• Any complaints about umpires or other incidents should be submitted to INFO@RISEOFRIVAL.COM.
• Umpires will also be held accountable for their conduct during games.

Player, Parent, and Fan Conduct:

• Silence is mandatory when the pitcher toes the rubber.
• Verbal abuse towards umpires will result in immediate action.

Ejection Penalties:
The length of ejections will be determined by the UIC and/or Empires' Director/Assistant Director.
